We are embarking on a journey to a better, healthier you. Please confirm your agreement with the following:

1. Assessment: I need personal information (age, medical history, medications, etc.) to complete my nutritional assessment and design your personal nutrition care plan.
2. Plan Design: Your plan may include specific recommendations to change food choices, quantities, and consumption timing.
3. Supplements: We may suggest nutritional supplements based on medical requirements with your consent. You shall use due diligence in selecting brands and understanding associated risks.
4. Activities: Your plan may require keeping a daily food record or diary.
5. Measurements: Height, weight, and waist circumference may be recorded as part of the assessment.
6. Confidentiality: I will keep your health condition confidential. However, I may confer with your physician about your nutrition care plan and obtain relevant information from them.
7. Validity: Packages are valid for 2 months from the last meeting. Unused sessions forfeit after 2 months.
8. Fees & Refunds: Fees are charged on a package basis. Services not availed within the package period are forfeited. There are no refunds or transfers to friends/family.
9. Cancellations: Please provide 24 hours' notice for any cancellation or change of appointment.
10. Digital Risk: Communication via email/text carries inherent security risks. While we make every effort to secure information, it is not 100% guaranteed.
